BACKGROUND
Background
Guilford County Department of Health and Human Services - Public Health Division applied for and received an award from The Office of Rural Health in the amount of $21,983 for the Medication Assistance Program (MAP). This program serves adults 18 years of age and older who need assistance obtaining prescription medications to treat chronic diseases due to cost. Typically, these clients do not have prescription insurance and have an income less than 200% of the federal poverty level. They must have a medical home and be a Guilford County resident. Each person meets with a clinical pharmacist who reviews, dispenses medications and provides education as well as communicates feedback to the providers. The effective period of this award is 08/01/2021-07/21/2022.
Funds will be used to purchase prescription bottles and labels and to support ten (10) hours per week of staff time.
