BACKGROUND
Background
NC DHHS requires Public Health to conduct a Community Health Assessment (CHA) every three (3) years. 1n 2019, Public Health and other health, social and community partners collaborated to complete this required community health assessment. The goals of the 2019 CHA were to identify key actionable issues, potential points of leverage and services or program gaps to inform efforts to eliminate health disparities and improve population health. Public Health will now make the document and the findings public. This action also meets Health Department accreditation and hospital Affordable Care Act (ACA) requirements.
