BACKGROUND
Background
Guilford County Department of Health and Human Services - Public Health Division receives funds for its Family Connects program. This program provides home visits by Registered Nurses to all new mothers in Guilford County. Nurses provide support, education and early identification and referral of health and safety concerns. This grant primarily serves the families in High Point, targeting the 27260 and 27262 zip codes for easier access for the target populations. Public Health has been granted permission from Healthy High Point to use original and unspent funds for program-related supplies and materials.
