BACKGROUND

Background

Guilford County has contracted with Senior Resources of Guilford for many years to provide rural outreach programs.  In addition to the congregate meals program the county previously provided funding for a Medical Social Worker that assisted with the Community Health Response Program (CHRP) participants with needed resources and eligibility for Medicaid and Medicare. 

 

Approximately one year ago the Piedmont Triad Regional Council Area Agency on Aging requested that the County consolidate the In-Home Aide programs to be administered by the Social Services Division of the Guilford County Health and Human Services Department which eliminated the need for the Medical Social Worker.  This change was effective January 1, 2017.

 

Senior Resources of Guilford has requested that Guilford County continue to assist with the rural outreach programs in Colfax, Oak Ridge, Pleasant Garden, Southeast Guilford, Stokesdale and Summerfield.  The outreach programs include monthly congregate meals and other activities for older adults over 60 in the six rural communities.  The requested financial assistance from Guilford County is $19,600 for FY 2018.  Senior Resources requests reimbursement at the end of each quarter.  Funding for the outreach program is included in the County’s FY 2018 budget.  

 

Senior Services of Guilford County’s seven Rural Outreach sites meet once a month, and a hot meal and programmed activities are included. The menu includes: proteins, two vegetables, bread, dessert and drink. In FY 16-17, 605 unduplicated older adults participated in programmed activities and received a nutritious monthly meal through Senior Resources of Guilford’s Outreach Program.

 

A sampling of the programs and services provided in FY 16-17 include: educational programs that address the preparation of wills, trusts, and asset protection, Medicare and information about enrolling in Medicare Part D Prescription Drug Plan, nutrition education by NC A&T Cooperative Extension, fire safety provided by Oak Ridge Fire Department, security issues by the Guilford Count Sherriff’s Department, scam and fraud prevention, Tai Chi and other exercise programs, evidenced-based programming including Matter of Balance Fall Prevention and the Chronic Disease Self-Management Program, Container Gardening by the Cooperative Extension, entertainment including the Tarheel Country Cloggers, Senior America, pet therapy and canine coaching services to assist seniors and their pets live well together.
