BACKGROUND
Background
Guilford County Department of Health and Human Services-Public Health Division has received an award from the Foundation for a Healthy High Point in the amount of $86,213 to fund a young adult clinic in the High Point location. This grant’s primary focus is to provide access to preventive health care for young adults, particularly in the 27260 zip code of High Point. Young adults lose touch with primary care providers during the adolescent years. The young adults in the Greater High Point area, especially those residing in this zip code, have poor access to care for various reasons including: lack of insurance, providers that understand their needs, transportation, and knowledge to understand the importance of preventive care.
This grant will fund a new Nurse Specialist I position to work with this age group in the Public Health clinic in High Point and in the community to provide education on the importance of preventive care. This initiative will remove many barriers that young adults face to access preventive health care. The funding period for this grant is January 1, 2017-December 31, 2017 with a two (2) year renewal.
Funds will be used for staff time (approximately $70,735 incl. benefits) as well as supplies, equipment, advertising and drugs ($15,478).
