BACKGROUND

Background

Community Care of North Carolina (CCNC) is a nonprofit organization dedicated to improving the health and quality of life of North Carolinians through support of the patient-centered advanced medical home model and innovative programs for healthcare providers and patients. Guilford County Public Health (GCDPH) Primary Care clinics treat patients with diabetes and are active partners with CCNC.

 

CCNC, in collaboration with Abbott Laboratories (“Abbott”), aims to enhance diabetes care through point-of-care hemoglobin A1C testing. While select clinics have received Afinion analyzers for on-site testing, CCNC is providing GCDPH with funding specifically to purchase the necessary testing supplies. These supplies will allow Public Health clinics to perform hemoglobin A1C tests for 330 clients, providing same-day results that enable immediate counseling, medication management, and comprehensive diabetes care.

 

By focusing on the acquisition of testing supplies, this initiative ensures that point-of-care testing is fully operational and sustainable. The service will support improved health outcomes, enhance quality of care, and may generate additional revenue through care gap closures and more efficient coding for visits.

REQUESTED ACTION

Requested Action

Approve a budget amendment increase in Other Revenue by $5,000 and increase the appropriation to Health and Human Services - Public Health by $5,000; and approve the contract between HHS-Public Health and Community Care of North Carolina, in substantial form, to be effective June 1, 2025, for the term of one (1) year for the purposes of supporting A1C supplies.
