BACKGROUND
background
Guilford County DHHS - Social Services Division is requesting an amendment to an existing Adult Day Care contract with Emanuel Senior Enrichment Center. The contract and funding/case management is needed to support aged and disabled adults in Guilford County who are vulnerable and at risk for placement into a long term care living situation, and/or who are at significant risk for social isolation without the daily activities/supervision. Services also assist with supporting the personal independence of older adults and promoting their social, physical, and emotional well-being.

Guilford County Department of Health and Human Services (Social Services Division) currently contracts with three (3) Adult Day Care/Health agencies. Because of the increase in the need for Adult Day Care services, and as a result of the recent closing of a center, consumers were relocated to another agency to meet their needs, and funding must now be reallocated. At the beginning of the current year of the Emmanuel Senior Enrichment Center contract, the service cost was $83,667.10. Guilford County DSS must now amend this contract to $115,282.93, due to the reallocation of funding. Home and Community Care Block Grant (HCCBG) funding is being increased by $18,328.75 taken from In-Home Aide funding. An increase of SSBG funds ($5,324.38) taken from In-Home Aide funding. A written request and HCCBG Budget revision paperwork was submitted to the Piedmont Triad Regional Council Area Agency on Aging requesting permission to reallocate funding. The Social Services Division is not requesting additional County funds; this is all based on reallocation of funding (The Piedmont Triad Regional Council of Governments approved an increase in funding of $42,929 and the Board appropriated this increase on 02/18/2016).
