BACKGROUND
Background
Guilford County DHHS - Public Health Division has received a grant from The Foundation for a Healthy High Point towards expansion of the Family Connects Evidenced Based Nurse Home Visitation Program. This program provides nurse home visits to all new moms in Guilford County; providing support, education and early identification, and referral of health and safety concerns. Nurses with advanced knowledge and experience in public health, perinatal nursing and lactation support will conduct post-delivery visits, with follow-up, as needed. This grant will fund a new full-time Nurse Specialist position to primarily serve the families in High Point; targeting the 27260 and 27262 zip codes where the majority of low income minority families reside. This grant is for a two (2) year period beginning July 1, 2016 and ending June 30, 2018 for a total amount of $175,317. Public Health is requesting an increase of $87,659 to the Health & Human Services - Public Health Division budget, to reflect the portion of funds that will be received during FY 2016-17 and one new full-time Nurse Specialist position contingent on continued grant funding.

REQUESTED ACTION
Requested Action
Approve an increase of $87,659 in Other Revenues and increase the DHHS - Public Health Division FY 2016-17 budget by the same amount to reflect funds awarded from The Foundation for a Healthy High Point and approve the addition of one new full-time Nurse Specialist position contingent on continued grant funding. Grant funds will be used for salary and benefits for the new full-time Nurse Specialist position.
