BACKGROUND
Background
The Community Care Physician’s Network (CCPN) is a clinical integration network utilized by the Public Health Department for practice assistance and contract negotiation with Medicaid Managed Care Prepaid Health Plans (PHPs). The purpose of the Innovation Funding Grant is to assist practices in identifying and executing projects that align with Community Care of North Carolina’s (CCNC) charitable mission-to build and support enhanced community-based health care delivery systems that improve the health and quality of life for all North Carolinians.
This grant will support targeted improvements in practice capabilities, focusing on:
· Electronic Health Record (EHR) Upgrades - ensuring secure, efficient, and interoperable health information exchange.
· Patient Education Technology - enhancing engagement and self-management tools to improve patient outcomes.
· Diagnostic Tools for Point-of-Care Testing - enabling more timely and accurate clinical decision-making within practice settings.
· Funding will be used for the following Electronic Medical Record Enhancements
o $6,481 Online Scheduling with patient check in (7-8 schedulers) Online scheduling integrated with the EMR allows patients to book appointments directly through a secure online portal or mobile app. This system automatically syncs with the EMR’s scheduling module, reducing administrative workload and minimizing scheduling errors. Reduces wait times and paperwork at the front desk. Improves data accuracy and workflow efficiency. Enhances patient satisfaction through convenience and transparency
