BACKGROUND

Background

The Community Care Physician’s Network (CCPN) is a clinical integration network utilized by the Public Health Department for practice assistance and contract negotiation with Medicaid Managed Care Prepaid Health Plans (PHPs). The purpose of the Innovation Funding Grant is to assist practices in identifying and executing projects that align with Community Care of North Carolina’s (CCNC) charitable mission-to build and support enhanced community-based health care delivery systems that improve the health and quality of life for all North Carolinians.

This grant will support targeted improvements in practice capabilities, focusing on:

                     Electronic Health Record (EHR) Upgrades - ensuring secure, efficient, and interoperable health information exchange.

                     Patient Education Technology - enhancing engagement and self-management tools to improve patient outcomes.

                     Diagnostic Tools for Point-of-Care Testing - enabling more timely and accurate clinical decision-making within practice settings.

                     Funding will be used for the following Electronic Medical Record Enhancements

o                     $6,481 Online Scheduling with patient check in (7-8 schedulers) Online scheduling integrated with the EMR allows patients to book appointments directly through a secure online portal or mobile app. This system automatically syncs with the EMR’s scheduling module, reducing administrative workload and minimizing scheduling errors. Reduces wait times and paperwork at the front desk. Improves data accuracy and workflow efficiency. Enhances patient satisfaction through convenience and transparency

o                     $26,263 for AI scribe (11 providers) An AI scribe integrated into the EMR uses artificial intelligence to automatically transcribe, structure, and summarize patient-provider encounters in real time. Reduces the documentation burden on providers. Enhances accuracy and consistency in medical record keeping. Increases provider face-to-face time with patients and overall productivity.

o                     $6,468 for telehealth (11 providers) Telehealth functionality within the EMR allows virtual patient visits to be scheduled, conducted, and documented seamlessly within one unified platform. Expands patient access to care, particularly for those with mobility, transportation, or geographic barriers. Improves continuity of care with documentation and follow-up embedded in the EMR. Streamlines billing and compliance with telehealth coding and regulatory requirement

The grant award amount is based on an allocation of approximately $16.70 per attributed member, using March 2025 Medicaid Standard Plan PHP attribution data of 2,348 members, as reported by each PHP in the Beneficiary Files to CCNC. Total grant funding is calculated using this standardized attribution to maintain consistency and accuracy despite monthly fluctuations.


The funding will be deployed in accordance with CCNC’s established guidelines for Innovation Funding, ensuring:

                     Compliance with all federal, state, and local procurement and spending requirements.

                     Alignment with evidence-based practice improvement strategies.

                     Measurable impact on quality, efficiency, and access to care.
