BACKGROUND

Background

The Guilford County Sheriff’s Office received a three (3) year U.S. Department of Justice, Bureau of Justice Assistance grant to provide evidence-based, substance use disorder treatment related to opioids, stimulants, and other illicit drugs. The grant was titled the Comprehensive Opioid, Stimulant and Substance Use Program grant. The Guilford County Commissioners voted to receive these grant funds during their regular meeting on December 19, 2024. The funds were in the amount of $1,589,826.00.

 

The treatment outlined in this grant includes medication assisted treatment (MAT), contingency management, and motivational interviewing. The treatment also includes harm reduction activities and recovery support service engagement with pre-trial and post-trial populations. These populations include citizens in our community and those who will be reentering it from either our local Detention Centers or a State facility.

 

This treatment involves embedding social workers, peers, and persons with lived experience in order to increase the connection to treatment and support services including post-release follow up. The goal is to reduce the substance use disorder recidivism rate by 50% in Year 3.

 

In Spring 2025, an RFP was conducted and the contract for services was awarded to Mental Health Associates of the Triad (MHAT) and initial services began. Due to staffing changes in late 2025, MHAT concluded that the organization lacked the capacity to sustain the grant and formally ended the contract effective January 30, 2026.

 

To avoid service interruption, the County promptly initiated a replacement‑vendor process. An RFP was issued in accordance with county procurement procedures, and a new vendor was selected within two months-meeting the target of a minimal lapse. The new vendor will operate under a county‑owned case‑management platform, ensuring that all data remain the property of the County. Six (6) laptops have been procured for the contracted staff, and comprehensive policies and procedures are ready for implementation.

 

The U.S. Department of Justice (DOJ) and the designated Training and Technical Assistance partner were notified of the transition. Both parties reviewed and approved the corrective actions, and the DOJ authorized all necessary grant‑award modifications.

 

An RFP was conducted and UHS High Point OUD LLC dba Foundations Health - High Point was awarded the contract for the services to be provided. The program employs six (6) contract staff members who work in both Detention Centers. The six (6) positions for these staff members will be: a licensed clinical case manager, licensed clinical additional specialist, community health worker, qualified mental health professional, and two (2) certified peer support specialists.

 

Given the DOJ’s standard practice of granting no‑cost extensions, the project team anticipates requesting an extension for the award through September 30 2028 to accommodate for the delay.

 

The budget for this contract will work as follows: remainder of Year 1 and Year 2 $687,932 (April 1, 2026 - September 30, 2027).

Approve the contract with UHS High Point OUD LLC dba Foundations Health - High Point, in substantial form, for a period of two (2) years and six (6) months effective April 1, 2026 through September 30, 2027 with the option to extend the contract to September 30, 2028 contingent upon receiving a No Cost Extension from the USDOJ for an additional year.
