BACKGROUND
Background
The Adopt-A-Mom program provides access to healthcare for vulnerable populations, as well as health education, counseling, and resources focused on reducing infant mortality and promoting healthy families. For 33 years, the program has partnered with obstetric providers in Guilford County to offer prenatal care at a reduced cost for individuals who do not qualify for Medicaid and are otherwise uninsured.
Grant funding from the Guilford County Partnership for Children’s Smart Start program and from Every Baby Guilford nonprofit organization supports this essential program, serving approximately 260 individuals annually. A Medical Interpreter is integrated into the program to eliminate language barriers. Additionally, both the Medical Interpreter and the Adopt-A-Mom Patient Coordinator serve as patient navigators, assisting clients with accessing and utilizing community resource support. Guilford County originally received an award from Guilford County Partnership for Children’s Smart Start program in the amount $87,655 that was approved by the Board of County Commissioners on October 17, 2024. The $17,335 reflected in this agenda item is in addition to the original amount received for a total amount of $104,990 appropriated to the project for Fiscal Year 2025. The additional $17,335 are intended to support the program through June 30, 2025 and will be used to support programmatic costs.

REQUESTED ACTION
Requested Action
Approve receipt of additional funding for the Guilford County Partnership for Children, and authorize contract amendment, in substantial form in the amount of $17,335. Amend the grant project ordinance for FY25 Guilford County Partnership for Children Smart Start Adopt A Mom Grant to increase the appropriation to the project by $17,335 with $17,335 in Federal/State Funds.
